Clinical Observation on Digitalis Intoxication

Abstract
|
|
|
A clinical study was made on 24 cases of digitalis intoxication which were observed at the National Medical Center from 1970 to 1973 and following results were obtained.
1. Gastrointestinal symptoms were most seen in subjective symptoms of the patient and nausea and/or vomiting was most observed among them.
2. Increase of automaticity (71%) was much more observed than conduction disturbance on electrocardiographic findings.
3. Hypokalemia (below 3.5 mEq/L) was observed in 4 cases (16%), but only one case has
U-wave on electrocardiograph.
4. The effect of inderal was much better than KCl in treatment of digitalis intoxication.
5. Overdose of digitalis was seen in 17 cases (71%) and it was most frequent factor in classification of etiology. 12 cases (50%) were with relative overdose and 5 cases (21%) were with absolute overdose in total 17 cases.
6. Limiting conditions of the patient (7 cases; 29%) were second after overdose of digitalis as etiological factor. Thus, individualization and reevaluation of dosage according to the limiting conditions are very important concepts in considering the etiology of digitalis intoxication.
7. The use of digitalis preparations without close observation of the doctor or irregular visiting hospitals of the patient was thought to be most barrier to prevention of digitalis intoxication.
